Submitting Your Application / Renewal Application On-Line
Welcome! This web portal provides the official utility to apply, renew, and update Behavioral Health Division Substance Use Disorder Certified Provider Information for the Wyoming Department of Health. The application is a multi-step process, as described below.

Before You Begin: Obtain Username and PIN  It is important to keep your Username and PIN in a safe place to access the online application in case you do not finish the application on your first attempt. Also to renew state certification on an annual basis, in addition to updating your demographic changes as they occur:

  1. Step 1: Basic Demographics
  2. Step 2: Additional Demographics
  3. Step 3: Levels of Service
  4. Step 4: Submit Required Documents
  5. Step 5: Submit Required Disclosures
  6. Step 6: Confirm Your Application
